In order to get Plaguebearer in Borderlands 3, you must play on Mayhem Level 6 or above. You want to finish the main mission from the base game: "Hammerlocked".
After completing the mission and killing the Warden, you will be able to return to this map and farm the boss as many times as you like. It is an enemy that has a huge armor bar and a health bar underneath. You will need to use cryo or corrosive elements effectively to get down his armor and then use fire to finish the boss.
